PIAA Paperwork:

The attachment at the bottom of this page is a blank copy of the PIAA's required physical for athletics.

Physicals must be signed and dated by a physician after May 1st of 2024 in order to be accepted for the 2024-2025 academic year. 

All physicals should be uploaded to Family ID.
